Direct Care Support Professionals (DSPs)

DSPs work closely with individuals who have developmental or intellectual disabilities. They help build independence and confidence through personalized support and daily goal setting.

Direct Care Workers (DCWs)

DCWs provide personal care such as bathing, dressing, meal preparation, and mobility assistance. Their work ensures individuals can maintain their routines and remain at home safely.

Community Health Workers (CHWs)

CHWs offer health coaching, care navigation, and community connection for those living with chronic conditions. They empower individuals to manage their health and build better outcomes.

Nurses (RNs, LPNs)

Our nurses support higher-acuity needs with care planning, assessments, and clinical oversight to ensure safety, well-being, and continuity of care.

Home Health Aides (HHAs)

HHAs assist with personal hygiene and household tasks, helping to bridge the gap between clinical needs and daily living.

Certified Nursing Assistants (CNAs)

CNAs monitor health status and assist with essential functions such as mobility and grooming, especially for those in recovery or with physical limitations.

Physical Therapists (PTs)

PTs help individuals regain movement, reduce risk of injury, and build strength to support independence in their homes.
